Στη Βυζαντινή Αυτοκρατορία, ο Κατεπάνω ήταν ανώτατος πολιτικός και στρατιωτικός άρχοντας. Η υπ'αυτόν διοικητική περιφέρεια (καθώς και το αξίωμα και η δικαιοδοσία του) ονομάζονταν καπετανάτου
[Σημ. 1] ή κατεπανίκιον[1]. Πριν τη διοικητική μεταρρύθμιση των Κομνηνών ο όρος «κατεπανίκιο» δήλωνε το «μέγα θέμα» και στην ιεραρχική κλίμακα ο «κατεπάνω» βρισκόταν αμέσως μετά τον «δούκα»(δουξ > κατεπάνω > στρατηγός), ενώ αργότερα ο «κατεπάνω» εκπίπτει στο ισοδύναμο ενός «κόμητος», όντας ενίοτε απλά τοποτηρητής στο όνομα του αυτοκράτορα και τίθεται πλέον στην ιεραρχία μετά τον «στρατηγό» αλλά και μετά τους κατώτερούς του[2].
Ο όρος συναντάται σε περιοχές της αυτοκρατορίας, όπως στην Τραπεζούντα, έως και τον 14ο αιώνα[3].
