      Infantile pyknocytosis is diagnosed only by careful analysis of peripheral blood smear, which reveals numerous small irregular and contracted red blood cells with hyper-dense spikes (the pyknocytes), that progressively increase in number to peak at approximately 3–6 weeks postnatal age, and then spontaneously disappear (at an average age of 4–6 months).
