Slavikų šv. Onos bažnyčia – bažnyčia, tituluojama šv. Onos vardu, stovinti Slavikų miestelyje, Šešupės dešiniajame krante.
Jurbarko seniūnas kunigaikštis Mykolas Čartoriskis iki 1752 m. pastatė medinę bažnyčią, nors kai kur nurodoma, kad ji pastatyta 1754 m.[1] Ji buvo Jurbarko parapijos filija. 1795 m. įkurta parapija. Pirmasis jos klebonas 1795–1827 m. buvo dominikonas Rapolas Aleknavičius (palaidotas šventoriuje). XIX a. pradžioje veikė parapinė mokykla.
Bernardino kunigo Lauryno Tautkevičiaus rūpesčiu 1835 m. pastatyta dabartinė medinė bažnyčia (konsekravo vyskupas Povilas Strašinskis) ir klebonija. 1858–1888 m. klebonavęs Pranciškus Andriukaitis padėjo knygnešiams platinti draudžiamą spaudą. 1906 m. įsikūręs „Žiburio“ draugijos skyrius 1907 m. įsteigė biblioteką – skaityklą.
1914 m. rugpjūčio 13 d. vokiečių kariai sušaudė Slavikų vikarą Andrių Pesį. Po Antrojo pasaulinio karo bažnyčia suremontuota.
2016–2019 m. buvo atlikti remonto darbai: atnaujintos bažnyčios sienos ir vidus, pakeistas stogas, aplink bažnyčią paklotas naujas takas.[2]
Bažnyčia stačiakampio plano, bebokštė, su 2 bokšteliais. Šventoriaus tvora medinių statinių ir mūrinių stulpų. Šventoriuje stovi medinė 3 aukštų varpinė.
