Simav Çayı er ei elv i Anatolia i Tyrkia. Ho er 321 km lang og har eit nedslagsfelt på 22 400 km². I antikken vart ho kalla Makestos[1][2] eller Mékestos (gresk Μέκεστος)[3].

Simav spring ut i provinsen Kütahya og renn nord over Simavsletta i provinsen Balıkesir. Det ligg eit reservoar ved Çaygörendammen,[4] og vidare renn ho forbi Susurluk og møter Adirnas. I antikken var Mekestos ei sideelv til Rhyndakos (dagens Adirnas), men i dag dannar Simav hovudleia frå samløpet deira nær Karacabey og ned til Marmarahavet. På dette siste stykket tek ho imot Nilüfer Çayı (i antikken Odrysses).
Munningen av elva er vend mot øya Imrali, som er eit høgtryggleiksfengsel.
