He began his career as a youth player with Al Ahly but was released in 2009, joining Tala'ea El Gaish. He spent time on loan with Haras El Hodoud before joining Petrojet in 2013. He returned to Al Ahly in July 2016 and then displaced Sherif Ekramy; he is considered one of the best goalkeepers in Africa. he helped the club win 4 consecutive Egyptian Premier League titles seasons 2016–17, 2017–18, 2018–19 and 2019–20 and 2 consecutive CAF Champions League titles seasons 2019–20 and 2020–21, the following year Al Ahly qualified for the 2021-22 CAF Champions League final, but was defeated by Wydad. The next year Al Ahly won the 2022-23 CAF Champions League, beating Wydad in the final, so the team has been in the final for 4 consecutive years, winning 3 finals of 4, making El Shenawy the only player that has won the CAF Champions League three times as a captain.
He made his international debut for Egypt in March 2018 in a friendly match. He was selected as the starting goalkeeper for their squad at the 2018 FIFA World Cup, playing in their opening two matches.
