Behind the club is the Alkaloid company, which owns 100% of the club.[1] RK Alkaloid started competing in the Macedonian First League in the 2021–22 season and managed to win the league title in their first season, securing a place in the Macedonian Handball Super League for the 2022–23 season.[2] They finished 2nd in the 2022-23 Macedonian Handball Super League season and they competed in the EHF European League group stage for the first time in the season 2023-24.[3]
- In 2024 Alkaloid won the Cup final for the first time, beating Pelister .The Cup of Macedonia brought a super-dramatic final between Alkaloid and Eurofarm Pelister, which was decided by penalties. The regular part ended with a score of 22-22, and after the sevens, Alkaloid was better, which won 25-23 and won the first historical trophy in the Cup of Macedonia.
- Super Cup 2024 The handball players of Alkaloid historically won the Supercup of Macedonia, after defeating the champion Eurofarm Pelister with 30-33 (14-19) in the final in Gevgelija. The match in the "July 26" hall was won by the Skopje team, which imposed itself from the start and had a constant advantage. Kire Lazarov's boys had enough concentration and nerves in the key moments, so in the end they deservedly rejoiced.
