Durante il liceo, Hanna Hilton alternava l'attività di cheerleader al lavoro part-time presso un locale della catena Dairy Queen a Connersville, nello Stato dell'Indiana. Ha cominciato a fare la modella dopo avere accettato di posare per una linea di costumi da bagno e lingerie. Le foto vennero scattate dallo stesso fotografo che realizzò le foto per l'annuario del liceo.[2] Apparve anche nel video Girls Gone Wild Bests Breasts prima di essere scoperta. Un agente la notò online e la convinse ad andare a Los Angeles.[3]

Il suo primo servizio fotografico per adulti fu realizzato per il numero di dicembre 2006 della rivista Penthouse, dove figurò come Pet of the Month.[4] Decise di realizzare scene hardcore a causa del fidanzato di allora, un attore porno poi condannato per stupro[5] di nome Jack Venice, che rifiutò di ritirarsi dall'attività[3] divenendo suo partner nella prima scena uomo/donna della sua carriera realizzata nell'aprile 2008 per la serie Brazzers.[2] Nel maggio 2008 firmò un contratto esclusivo con la Vivid Entertainment.[2] Il suo film di debutto per quella compagnia fu Meggan and Hanna Love Manuel.[4]

In un'intervista pubblicata nel numero di settembre 2008 della rivista Xcitement Magazine, Hanna Hilton ha riferito di avere un figlio di quattro anni dall'attore porno Jack Venice.[6]

Il 28 settembre 2009, LukeFord.com ha riportato la notizia che Hannah Hilton ha abbandonato l'industria pornografica.[7]
